Health care services are often disconnected. Having a primary care provider who can take the time to bring it all together and see the big picture has become a distant memory. Many health services are available, but it can be difficult to find and understand them. It can be challenging to advocate for yourself and get what you need.

Health navigation begins with a very thorough assessment to explore your health history, self-care practices, current symptoms and concerns, and most importantly - your health-related goals. Together, we will develop a plan to optimize your health. You will feel prepared and supported to integrate and engage both “nonmedical” and “medical” services to meet your health goals.

Health Navigation Services

  • As we age, our bodies' ability to metabolize (utilize) medications and excrete them decreases. Each additional medication multiplies the likelihood of side effects due to interactions and competition for crucial limited physiological services (such as albumin binding, liver enzyme processing, and nephron excretion). It is common for individuals to be taking medications that are no longer helpful (or outright harmful) because a comprehensive medication review is time-consuming and is not a strong skill of all providers.

    If desired, we can work together to review why each medication is (or is not) important for you. Based on your needs, goals, and preferences, a report will be generated and sent to your prescriber(s) with recommendations for stopping and/or reducing medications that no longer fit your health goals or needs. All recommendations include rationale. The prescriber will then work with you to make changes as appropriate.

  • Have you ever left a visit with a specialist and felt like you have more questions than answers? That they were focused on the wrong problem? That you simply didn't understand what they were telling you?

    We can work together to prepare for your upcoming specialist visit(s). We will identify your goals and questions, conduct preliminary research, and ultimately prepare an outline to bring with you to the office visit. The outline will include, in clear terms, your goals, questions, constraints and concerns. You will be prepared to record what they tell you and identify next steps following the visit. Providers will be delighted with your preparation and readiness to learn, and can more effectively help you with the limited time they have available for your visit.

  • There is an unfortunate divide between the medical (surgical + pharmaceutical) specialties and holistic practices such as massage, acupuncture, and energy work. Fortunately, there are practitioners who bridge the gap, such as Medical Massage Therapists, Functional Medicine Practitioners, Naturopaths and Chiropractors. Smart health consumers are familiar with the different services available, and can quickly find the right help at the right time. Let's work together to optimize your health care team.

  • Is your body acting strangely, and no one can seem to give you a real answer of why or what to do about it? Common doctor-baffling symptoms are pain, gut issues, brain fog, and fatigue. With a very thorough assessment and research that follows, we can find answers to what is going on with your body. It could be an infection (such as Lyme disease), a neurologic autoimmune disease (such as Guillian Barre syndrome), We'll work to get the right team together to optimize your health.
